Position Overview
We are seeking a reliable and organized Administrative & ; Case Management Assistant to support daily office operations and resident coordination for our group home and home care agency. This role is critical to ensuring residents receive necessary services while maintaining organized records, smooth operations, and timely communication.
Key Responsibilities
Resident Case Management
Ensure each resident has required documentation (ID, Social Security card, birth certificate)
Assist residents with enrolling in eligible services and benefits
Schedule and track medical appointments
Coordinate transportation for appointments and services
Maintain accurate and up-to-date resident files
Complete and manage resident onboarding paperwork
Monitor resident needs and follow up on outstanding items
Group Home Operations Support
Conduct and document routine cleaning and maintenance checks
Ensure daily operational standards are met within each home
Communicate maintenance issues and coordinate resolutions
Support house managers with operational needs
Office & Administrative Duties
Organize and maintain physical and digital files
Answer incoming calls and respond to emails professionally
Handle resident and family grievances and route issues appropriately
Assist with documentation for the group home and home care agency
Support general office tasks as needed to maintain efficiency
Communication & Compliance
Serve as a point of contact for residents, families, service providers, and staff
Maintain confidentiality and compliance with all privacy requirements
Document interactions, grievances, and resolutions accurately
Qualifications
Strong organizational and time-management skills
Excellent written and verbal communication
Experience with administrative work, case management, or healthcare support preferred
